How to Apply for PhilHealth Membership

Duly accomplished registration form
Government issued ID
Supporting documents for dependents:
- Registrant:
  - Government issued ID
  - Current employees ID with the signature of the employer or authorized representative;
  - Postal ID;
  - Students ID or library card, signed by the school authority;
  - Senior Citizens ID;
  - Driver’s license;
  - NBI/PNP clearance;
  - Passport;
  - SSS/GSIS ID;
  - Integrated Bar of the Philippine (IBP) ID;
  - License issued by the Professional Regulatory Commission (PRC)
Spouse:
- Marriage Certificate
Children below 21 years:
- Birth Certificate
Adopted Children below 21 years:
- Court Decree of Adoption
Stepchildren below 21 years:
- Marriage Certificate of the natural parent and the stepparent and Birth Certificate of the stepchild
Children below 21 years with disabilities:
- Birth Certificate and Medical Certificate
Parents over 60 years old:
- Birth Certificates of the registrant and the parent
Stepparents over 60 years old:
- Marriage Certificate between the natural parent and the stepparent and Birth Certificates of the Stepparent and the registrant.
- If the natural parent is deceased: Death Certificate of the natural parent
Adoptive parents over 60 years old:
- Court Decree of Adoption or photocopy of the registrant’s Birth Certificate in which the adoption is annotated
- Birth Certificate of the adoptive parents or a notarized affidavit of the date of birth
