How to Apply for PhilHealth Membership

  • Duly accomplished registration form
  • Government issued ID
  • Supporting documents for dependents:
    • Registrant:
      • Government issued ID
      • Current employees ID with the signature of the employer or authorized representative;
      • Postal ID;
      • Students ID or library card, signed by the school authority;
      • Senior Citizens ID;
      • Driver’s license;
      • NBI/PNP clearance;
      • Passport;
      • SSS/GSIS ID;
      • Integrated Bar of the Philippine (IBP) ID;
      • License issued by the Professional Regulatory Commission (PRC)
  • Spouse:
    • Marriage Certificate
  • Children below 21 years:
    • Birth Certificate
  • Adopted Children below 21 years:
    • Court Decree of Adoption
  • Stepchildren below 21 years:
    • Marriage Certificate of the natural parent and the stepparent and Birth Certificate of the stepchild
  • Children below 21 years with disabilities:
    • Birth Certificate and Medical Certificate
  • Parents over 60 years old:
    • Birth Certificates of the registrant and the parent
  • Stepparents over 60 years old:
    • Marriage Certificate between the natural parent and the stepparent and Birth Certificates of the Stepparent and the registrant.
    • If the natural parent is deceased: Death Certificate of the natural parent
  • Adoptive parents over 60 years old:
    • Court Decree of Adoption or photocopy of the registrant’s Birth Certificate in which the adoption is annotated
    • Birth Certificate of the adoptive parents or a notarized affidavit of the date of birth
