How to apply for Ombudsman Clearance- Tutorial

Retiring government officials and employees may apply online for Ombudsman Clearance. Ombudsman Clearance is an official document certifying that a person has no criminal, administrative, and forfeiture case/s pending with the Office of the Ombudsman.

  1. Any person may file an application for his or her own Ombudsman Clearance personally or through a duly authorized representative
  2. Heads of departments, offices, agencies, bureaus, or their duly authorized representatives, with respect to their own personnel or under the terms of existing memoranda of agreement
  3. Chairpersons or duly authorized representatives of the Judicial and Bar Council, Commission on Appointments and other search and selection committees or bodies, as to their respective applicants/nominees

REQUIREMENTS
  1. Duly accomplished Application for Ombudsman Clearance (If filed by the applicant personally or through a duly authorized representative)
  2. Formal request addressed to the Ombudsman (If filed by those enumerated in A other than applicant or his duly authorized representative)
  3. Certified true copy of the Service Record reflecting the date of retirement/resignation, or, where, applicable, the personal data or information sheet
  4. Government-issued ID of the applicant/duly authorized representative
  5. Payment of the processing fee, as may be determined and authorized under pertinent issuances
FEES
Office Circular No. 07 (series of 2014) set the schedule of processing fees which range from:
  • P100.00 (for retirement purposes, rationalization, resignation and death claims)
  • P150.00 (for employment, promotion, transfer, appointment, confirmation, discharge, foreign assignment and grant of benefits unrelated to the preceding purpose)
  • P200.00 (for foreign travel, foreign assignment, study grant/scholarship, recognition/awards, leave application, loan application, change of name, and as part of documentary requirements of the JBC, CESB, CSC, PRC and other agencies)
  • P500.00 (for permit to carry firearms, fidelity bond and bidding requirement)
  • P1,000.00 (for firearm licenses).
PROCEDURE
  • An application or request for issuance of Ombudsman Clearance may be filed at any time, however, for retirement purposes, an application shall be filed not earlier than six (6) months prior to the date of retirement of the applicant.
  • Please write N/A for each entry that does not apply to you.
  • Moreover, this online application is made under oath. Make sure that all information provided are true and correct to the best of your knowledge and is supported by proper and adequate documentation, and you must be prepared to produce the same when required in the future.
  • No Ombudsman clearance is necessary for the payment of the money value of the accrued leave credits (Terminal Leave Pay) of a separated official or employee, subject only to whatever claims the paying agency may have against the same (Per MC #5 Series of 2012, signed by Ombudsman Conchita Carpio Morales).
  • For retirement/resignation/separation purposes, please email your service record reflecting the date of retirement/resignation/separation to ombclearance@ombudsman.gov.ph.
  • For death claim purposes, please email the service record and death certificate of the deceased toombclearance@ombudsman.gov.ph.
  • This online facility caters only to applications filed individually and will not accept bulk applications.

  • Any SM branch
  • Postal money order payable to the ‘Office of the Ombudsman Clearance Fees’ directly to the Office of the Ombudsman, Agham Road, Diliman, Quezon City
MODE OF RELEASE
  • Release of clearance/certification will be done within five (5) working days from receipt of payment either:
    • For pick-up
    • By ordinary mail which is free of charge
    • If you prefer to receive the clearance/certification earlier, you may send a prepaid envelope from any private courier
  • An official receipt shall be attached to the duly issued Ombudsman Clearance or Certification.
